The Neurological Alliance of Scotland is carrying out a survey of the most recent experiences of patients with neurological conditions when they've accessed NHS services – other than visits to their GP.
The aim is to give the Neurological Alliance ammunition to fire when discussing improvements required to local adult NHS neurology services.

If you're over 18 and have M.E. or Chronic Fatigue Syndrome – or indeed have suffered from Post Viral Fatigue Syndrome for some months – please take the time to fill in the form. The Neurological Alliance want to hear about your most recent encounter with any specialist service in the last 12 months.
The deadline for completion of the forms is Friday, October 14. It contains 15 questions which you should be able to answer in one sitting but, if you can't, try saving the form and come back to it later.
