“The Community Neuro Rehabilitation Team (CNRT) is a clinically-led neurorehabilitation service for those with an acquired brain injury, stroke, neurological condition or confirmed diagnosis of chronic fatigue syndrome.
“The Community Neuro Rehabilitation Team provides the following service to patients:

  • Individual assessment and therapy on an outpatient basis
  • Specialist assessments of function, cognition, speech and language, swallowing, psychological adjustment, mobility
  • Group programmes for chronic fatigue syndrome
  • Group programme called “Regaining Confidence after Stroke”
  • Other groups include upper limb group, balance group, cognitive rehabilitation, baking group and fatigue management for multiple sclerosis. Patients access these in line with their goals
  • Multidisciplinary case management through key workers to ensure coordinated care
  • Support and training for relatives and carers
  • Follow-up support for those admitted to hospital following a head injury
  • Consultancy

“Patients can be referred to the service by their GP, medical consultant, other health professionals, social worker, external agencies or charities. Patients can self-refer unless they have a diagnosis of CFS which requires a medical referral to ensure that the diagnosis has been confirmed.”
