Firstly, is there any guidance and research into low ferritin levels and ME?
Secondly, is it worth taking an iron supplement to increase energy levels even if serum ferritin levels are normal and there aren’t any symptoms or signs of iron deficiency anaemia?
Thirdly, are there any risks to taking one of the commercial iron supplements that you can buy over the counter in a pharmacy and that are advertised for treating fatigue?
Answer
The serum ferritin blood test measures the amount of ferritin in the blood. Ferritin is a protein that carries iron around the body – so the blood level provides a very sensitive guide to iron deficiency and iron overload.
Serum ferritin is one of the raft of blood tests that should always be checked before a diagnosis of ME/CFS is made. The ferritin level should be normal in ME/CFS – as the disease process does not cause iron deficiency, or iron-deficiency anaemia. However, if you are on a diet that is low in iron-containing foods this could cause a lower than normal level.
Consequently, there is normally no need for people with ME/CFS to take iron supplements, especially if their ferritin level is normal and there are no other indications of iron deficiency anaemia.
If the ferritin level is reduced then the cause needs to be established. If an iron-deficiency anaemia is present, iron supplements will be required. There are various options to choose from (tablets, injections) but they should only be used under medical supervision. Taking iron supplements when there is no need to do so can cause serious side effects as a result of excess iron being deposited in parts of the body where it should not be.
