“Building Bridges in Healthcare: An Inspiring Model for Interdisciplinary Care in Wales”

Last week, a member of the Health and Social Care (HASC) team met with a consultant therapist co-lead from the Chronic Complex Conditions Service within a large Welsh Health Board. Their team has built a truly interdisciplinary service from the ground up, including Advanced Clinical Practitioners (with independent prescribing rights), physiotherapists, Occupational Therapists, dieticians, clinical psychologists, GPs, and Speech and Language Therapy specialists. Importantly, they offer tailored care for severe and very severe patients and operate on a needs-led rather than diagnosis-led approach. The lead is eager to foster collaboration across all Welsh Health Boards to develop a best-practice model of care. We are extremely grateful to them for sharing their story, and we will stay in touch to learn from their ongoing progress.
